CB360 Universal Kill switch / turn signal wiring

I bought the CMC Universal Kill Switch (part no. 2028), and I'm not sure how to wire it up. It has 3 wires coming out of it, I know the black/white wire is for the running circuit, I'm just not very experienced in wiring and am not sure how to make it so it works like the original off/on/off. I am also using one as my turn signals, does the switch use a ground or does the ground come from the signal itself? And lastly, I have a 3-way headlight toggle switch (part 3052), and I need some insight on how to wire that. I know one prong will be the low and one will be the high, but do I need to ground the switch, or does another wire come in as the "hot" wire on the third prong? Thanks.
